关心 is not optional
If you look unwell in China, somebody will say so and then do something about it. 关心 — concern, care, attention paid to another person's condition — counts as an obligation of friendship rather than an intrusion on privacy, so a colleague who notices your face will ask, press for detail, and produce advice or medicine. Declining once, politely, does not end it. The register can feel intrusive to someone raised to keep such things private; read it as the opposite of intrusion, and accept the hot water.
