Which scheme you are in, and the things it quietly includes
Health insurance in Germany is compulsory, and which of the two systems you belong to is decided mostly by your income rather than your preference. Employees below an annual income threshold — the Jahresarbeitsentgeltgrenze, adjusted each year — must be in the statutory scheme. Above it, and for the self-employed and civil servants, private cover becomes possible. The contribution is a percentage of your gross salary, split with your employer, so it rises and falls with what you earn rather than with your age or health. Within the statutory scheme you choose your fund freely, and this is the choice Alex is making. The core benefits are set by law and therefore nearly identical everywhere; what differs is the Zusatzbeitrag each fund levies on top, plus service, apps, and extras like additional preventive care. Switching is straightforward after a twelve-month commitment, so a bad choice is not permanent. Two features of the statutory scheme that private cover does not replicate, and that newcomers often do not know. Family members without their own income — a spouse, children — are covered at no extra cost under the Familienversicherung; private policies charge per person. And your Zuzahlungen are capped at two per cent of annual income, one per cent for chronic conditions, after which you can apply to be exempt for the rest of the year. Keep the receipts. The warning worth stating plainly: moving from private back to statutory is difficult and in middle age often impossible. Private premiums are calculated on age and health at entry and rise steeply later in life, which is why the arithmetic that favours it for a healthy thirty-year-old can look very different at sixty. It is the one decision here that is hard to undo, so it is worth advice rather than a comparison website.
